Hal puts Maxvill in for Milan. Milan was 2 e16. Maxvill 3e9. But I thought the 2 range was more important than a slightly worse e rating. No injury. No double switch. Do I have it wrong?

HAL did a double-switch to prevent Stu Miller from batting in the top of the ninth, not because Maxvill is a better fielder than Milan.

Was not a double switch. Maxvill batted in Milan’s slot.
If it where a double switch Maxvill would have batted in 9th spot

Yes, you are right. Seems totally unnecessary to bring in Maxvill at that point. Should have kept him on the bench. Perhaps, HAL compared Milan's fielding percentage (.900) on your team to Maxvill's fielding percentage and made the decision based on that fact. Wouldn't want HAL to do that, but, maybe he makes decisions based on simulated league result rather than real-life result. I once had the misfortune of having HAL pinch-hit for Cal Ripken in the post-season even though he hit over .300 in the regular season and I had his best card. He was batting something like .196 in the post-season, so HAL lifted him. I put an end to that by making sure Ripken wouldn't be pinch-hit for the rest of the way.

My guess is it's a logic bug. HAL says "I'm doing a double switch when I bring in a relief pitcher whether it makes sense or not. I don't care if I can't find a suitable sub for Norm Cash or Pete Ward. Somebody's coming out of the game besides the pitcher because I've already made up my mind." before he tries to execute the action.

They don't call it a mystery game for nothin'.

Hal has real issues with a double switch--I have seen that issue time and time again--I prefer noDH myself, but over the time I have played online I have switched to playing mostly DH leagues simply because the AI is inadequate with the PH for pitcher then the ensuing double switch.
